Thank you for considering Advanced Pain Care. We are currently accepting new patients and have short wait times for appointments. Once we receive the necessary medical records, we can promptly schedule an initial assessment at our office. We kindly request that you obtain a referral from your primary care provider (PCP) or the healthcare professional currently overseeing your pain management.

If you are experiencing chronic pain, many providers are willing to fax us a referral request and copies of your medical records. Required Documentation

To help expedite your appointment, please provide us with the following documents that can be faxed, mailed, or hand-delivered to our office. Remember, the sooner we receive these records, the sooner we can schedule your appointment. We advise our patients to be polite and persistent in obtaining their medical records from clinics and hospitals, as it often helps speed up the process.

  1. Referral request from your PCP or current pain management provider. 

  2. Copies of the last few months of medical records from your current pain management provider. We don’t need an extensive history; what we value most is a complete, accurate, and up-to-date medication list with dates and dosages prescribed. 

  3. Copies of reports from any diagnostic imaging or testing that has been done to diagnose the pain problem (MRIs, CT scans, x-rays, ultrasound, electromyograms, nerve conduction studies, discograms, etc.). Actual images are not necessary; the reports of findings from the interpreting doctor work best for us. 

  4. Copies of procedure reports or medical records from any surgeries or other procedures or injections that have been performed for the pain issue. 

  5. Complete insurance information (if you have insurance), including the insurance company name, group number, subscriber ID number, subscriber name, and deductible and copay information. Patients without medical insurance or with medical insurance that we don’t accept can still beseen at our office. We offer a discounted rate for self-pay. Please note that payment is due at the time of service. 

  6. A referral from your PCP to authorize benefits, which some insurance companies require. It is your responsibility to obtain this authorization if you want your insurance company to cover your care with us.

Kindly check the list of our currently contracted insurance providers before scheduling your appointment to verify coverage. If your HMO or PPO doesn’t have an in-network pain doctor, you can usually receive authorization from them to receive out-of-network benefits when seeing us.
